36M suffering from chronic back pain. (MRI attached)

 36M Patient feeling hopeless and afraid of problem getting more serious in future. 


In 20s he have worked in job involving heavy weight lifting.  He had minor back pain problems starting in 2004 but now its increased since 2-3 years and since 1 year pain have increased much more. He is unable to lift any heavy Weight now, currently his job involves motorbike driving for 3 hours a day but nowadays he is on holiday since 2 months. 



Now he is unable to lift any heavyweight and he avoids it, recently he felt difficult to walk one day. When he sits for long hour then it becomes very difficult to stand up. 


What can be done for him? 


Physiotherapy, exercise, lumbar support belt, had been advised in past but he didn't followed any regularly but now want to.. He was recently advised Steroid injection by a local quack as a possibility but patient afraid of it and wants to avoid if possible to improve without it. He knows surgery is an option at very late stage and believes he can follow preventive steps.
